Concierge Fee Schedule
Unlimited medically necessary visits, prescriptions, lab / blood work orders , and imaging orders. 24 hours per day six days a week access to medical provider via email/call/text, and same or next day appointments included. This is our preferred plan for clients wanting executive level access to their provider and for those that are uninsured.
$1,500 per year for a single person
$900 per year for a single person —-telehealth only— any home visit that is needed on this plan will cost an additional $70 per visit
Two-person family $2,600 per year
Two person family —telehealth only— $1,600 per year, any home visit that is needed on this plan will cost an additional $70 per visit
Additional $720 per year per child added
For small businesses (up to 30 employees): $50 per month per employee
Concierge service is not required to be a patient or receive care.
